I don't think he means related as in connected. It's clear that everyone in this show has some sort of connection with each other. I think he means it's confusing to find out every other episode that someone is someone else's sister or mother or uncle, etc.

He speaks English when he's with English speaking people. But he and Ando are Japanese, so it's normal they speak their own language between each other. One of the great things of the show is that not everything happens in America and not everyone on the planet speaks English. It's just more realistic. If you were in Japan with a friend of yours, would you be speaking Japanese of English with her/him?
